The Clean(ish) Skincare Routine That Finally Stopped a Six Month Rosacea Flare
Rosacea is exhausting. Most of the time, my skin is flushed, reactive, and reminding me that I am not the one in charge. My most recent flare lasted more than 6 months and came with a lovely bonus: persistent acne. Rosacea acne doesn’t have a whitehead, it just feels like a mountain underneath your skin.
It felt like nothing I tried was helping so I mostly gave up. I occasionally tried new clean beauty products, hoping they’d be gentle and effective, but nothing was working. So I ended up under-treating my skin trying to not make it worse. I cleansed while hoping for the best. I avoided exfoliation because I couldn’t find anything clean and gentle enough. And then there were the moisturizers I didn’t really care for. I wore a lot of makeup to try to cover the redness, but it never could cover the rosacea texture.
I had nearly given up on finding a routine that worked.
Finally, one evening several weeks ago, while researching perimenopause, I ended up on MyAlloy.com. When I saw their prescription rosacea cream, I recognized every active ingredient: azelaic acid, metronidazole, and ivermectin. I had used all 3 before at varying times but never compounded and not even simultaneously. It made all the difference. Within days of starting it, my skin started calming.
Around day 3, I saw less red in the mirror. Within a week, the bumps were healing and disappearing. Two weeks in, I could leave the house without makeup. My skin was still pink, but it was a tremendous improvement.
Once the champion product was identified, I felt confident in my ability to find other gentle products to round out a full yet simple routine.
And here’s the part you may find surprising: with one exception, every product in this routine is low‑hazard as rated by the Environmental Working Group (EWG) (rated 1 to 2 out of 10, which is in the green [aka, low hazard]), even though none of them are strictly clean beauty. They’re all simple, fragrance‑free, non‑irritating, and built around ingredients that have been studied for years. Most of them are single‑ingredient formulas or straightforward blends centered around potent actives.
Here’s the exact routine that finally calmed my worst flare ever over the past month and have become the staple items I won’t ever be without.
Morning Routine: Calm, Protect, and Keep It Simple
Alloy Rosacea Face Cream
This prescription cream was the turning point. It targets inflammation, redness, and bumps. Even though it’s prescription‑strength, the individual ingredients are all well‑studied, effective, and gentle.
Azelaic acid is one of the most rosacea‑friendly ingredients out there. It’s so safe, it’s even safe during pregnancy. Metronidazole has decades of research behind it and people don’t build immunity to topical antibiotics. Ivermectin helps calm inflammation and those deep, painful bumps.
I have used all 3 ingredients separately at different times for different flares and saw modest improvements.
This is wildly different. It only takes one pump to cover my whole face. It goes on smoothly and the formulation is pleasant. I especially like that I don’t have to layer products. It simplified my routine and strengthened it at the same time.
EltaMD UV Physical Broad-Spectrum SPF 40 (Rosy Tint)
Tinted moisturizer is a nightmare on very fair skin. It turns orange, it’s impossible to look natural, and it leaves a line of demarcation between your face and your neck. If you know, you know. But this one is a unicorn. It’s zinc‑based, gentle, and the tint is rosy instead of tan. (There is a more sun-kissed tint as well.) It blends in without effort and gives a mattified, soft finish that tones down redness.
This is the lowest rated item in my routine, with a 3 (moderate) rating, based on the way they score sunscreen. But most of the concern is regarding allergies I don’t have.
Night Routine: Reset, Repair, and Keep the Barrier Happy
Neutrogena Hydro Boost Cleansing Gel
A gentle, no‑nonsense cleanse that doesn’t strip my skin or leave it tight. It removes sunscreen without scrubbing, which is essential when your face is already irritated. And the gel texture feels soothing instead of squeaky or harsh. It’s fragrance‑free and simple in the best way: no actives, no surprises, just a cleanser that respects your barrier. After months of overthinking every product, having a straightforward, dependable wash step has been a relief.
The Ordinary Mandelic Acid
Physical exfoliants are out of the question for me, and even chemical exfoliants are usually too much. This is the only exfoliant my skin will tolerate anymore. Mandelic acid is the gentlest AHA, and The Ordinary’s formula is super clean, super simple, and somehow manages to smooth texture without burning. Most importantly, it doesn't make me more red afterwards. I use it 3-4 nights a week.
The Ordinary Acne Spot Treatment
This is my emergency brake for random breakouts. It’s also the only item in the lineup I’ve been using continuously for years. It’s a spot treatment for occasional breakthrough acne. Sometimes it’s able to dry up trouble spots with one application. And like most of The Ordinary’s products, it’s a simple, single‑active formula. I’ve also used Kate Somerville’s Acne Spot Treatment, which is basically the same thing. I skip mandelic acid on the nights I use this to avoid over-exfoliation.
CeraVe PM Facial Moisturizing Lotion
I’ve tried so many moisturizers over the years. The ones that sting, the ones that feel heavy, the ones that feel like they’ve evaporated, the ones that actually cause breakouts. I actually used CeraVe PM years ago for a while and loved it, but I kept wandering off in search of the mythical clean holy‑grail moisturizer. At some point I realized I didn’t need perfection. I need something clean enough that worked that I loved using. So I came back to this, and honestly, it’s a relief.
It’s lightweight, non‑oily, and layers beautifully. The formula is simple and pleasant to use. I prefer a pump bottle for getting the right amount of product, which this has. The few flagged ingredients aren’t sensitivities I personally have, which makes it an easy yes for my skin.
The Moment I Knew the New Routine Was Working
About a week into the new routine, there was a quiet moment of catching my reflection and realizing my cheeks weren’t glowing red anymore. I did a double-take. My skin was not perfect but it was noticeably calmer. After months of dealing with my angry skin, seeing that softness in the mirror was a relief.
Since then, I’ve continued to see improvement, and my skin is finally smooth again after months of rough texture and bumps. Now I wake up without any redness. As I write this nearly a month in, my skin has the vaguest amount of pink and no new bumps.
A Routine That’s Actually Sustainable
This routine is sustainable not just for my skin but for my budget as well. Every product is affordable and lasts a long time because the formulas are simple and built around potent actives instead of long ingredient lists.
Here’s the cost breakdown if I repurchased everything every 90 days (in reality, the spot treatment will last at least double that and probably the same for the mandelic acid):
| Product | Cost | Notes |
|---|---|---|
| Alloy Rosacea Cream | $100 | Prescription; does the heavy lifting |
| EltaMD UV Physical SPF 40 | $43 | Mineral-only, sensitive-skin friendly SPF |
| CeraVe PM Moisturizer | $15 | Lightweight, barrier-supportive moisturizer |
| Neutrogena Hydro Boost Gel Cleanser | $12 | Gentle, fragrance-free cleanser |
| The Ordinary Mandelic Acid | $8 | Simple, single-active exfoliant |
| The Ordinary Acne Spot Treatment | $10 | Fast acting emergency treatment |
Total: $188 for 90 days, which is just over $2 per day. The prescription cream is by far the most expensive product, but I’m able to purchase it using HSA/FSA tax-free dollars. Everything else in the routine is simple, gentle, and budget‑friendly.
A Word for Anyone Walking Through Their Own Rosacea Season
If you’re going through your own rosacea flare I hope this gives you a little hope. You’re not alone in it and you’re not doing anything wrong. Sometimes it just requires finding the first right thing to get you going in the right direction. Once I had my foundational treatment I was easily able to finish out a low maintenance, sustainable beauty routine built around my sensitive, rosacea prone skin.
Full disclosure: These are my opinions of products I use and love. I was not compensated for any review.
